Instruction Through Multiple Pathways for Adult Community Transition (IMPACT)

Students who are referred for community-based transition services are:

  • 18-21 years old, reside within the boundaries of Adams 12 Five Star Schools

  • Have an active Individual Education Plan (IEP)

  • Have not received a high school diploma from any school district AND

  • Require additional time to meet post-school transition goals based on identified needs

Referrals should be made by the first quarter of the student's senior year

IMPACT Requirements

Students receiving IMPACT Transition Services, for reasons of safety and effective instruction, must able to demonstrate the ability to:

  • Understand and respond to prompts, redirection, and positive reinforcement

  • Maintain safe and positive behavior appropriate for the community environment and refrain from behaviors injurious to self, others, or property

  • Benefit from decreasing levels of direct adult support in community settings

  • Be unsupervised at times in home and/or community

  • Become independently mobile in the community using family, RTD or Access-a-Ride transportation services, or by driving

  • Independently perform basic activities of daily living, such as dressing, eating, and toileting

  • Verbalize or effectively use alternative systems to communicate needs
