Information Release
Adams 12 Five Star Schools discounts or waives certain student fees based on a student's free and reduced status. This status is protected by USDA government regulations; therefore, in order to have student fees discounted or waived, parents must sign an information release form that gives permission for Adams 12 Five Star Schools to verify a student's free and reduced status. This can be achieved two ways:
- Sign the Information Release form as part of your online application (easiest method), or
- Print the Information Release form (below), fill it out, and return it to any district school office.
Parent/Guardian(s) can also show the final meal benefits approval letter they receive from Nutrition Services to any district school to have fees waived or discounted.
