New Employee Administrative Orientation Information
Click here to sign up for an Administrative employee orientation session. New Hire Orientation is approximately 3 hours.
The first part of orientation will consist of reviewing hiring payroll forms and reviewing school district policies and procedures.
If you are a current employee with School District 12, you will not need to provide additional identification or fill out the hiring payroll forms as requested below.
Administrative Orientation Checklist / New Hire
Identification is required at an orientation session. Your IDs will verify your eligibility to work. Please click here to see a list of acceptable forms of identification. Some of the most common documents are listed below.
Combination 1: Unexpired Passport (no other documents are needed with a passport)
Combination 2: Driver's License (or State ID Card) and Social Security Card
Combination 3: Driver's License (or State ID Card) and Birth Certificate
Please complete all items on this page prior to your orientation session.
New Employees - Please download, complete and bring each of the following forms to orientation. (We will be discussing these forms during orientation with questions that you may have.) These forms MUST be completed prior to orientation. Most of these forms are form filled and your information can be typed onto them. (Coaches- Please use classified link). Thanks

- Administrative Non Certified Multi-Form
- Administrative Certified Multi-Form - Has an administrative license.
If you are a non-certified Administrator and are required to get a fingerprint card completed, please use this list for Fingerprinting Agencies.
Official transcripts are required at the time of orientation. If you do not have an official copy available, please order a copy for the Human Resources department.
PLEASE NOTE: As of July 1, 2012 – all Classified and Administrative Employees will start contributing an additional 1.5% to PERA. The 1.5% will be taken from earnings as a reduction prior to generated monthly gross earnings. Additional contribution to be reflected in monthly pay advices beginning with the July 2012 Pay Advice.Attn: Linda Roth
