Parent Special Ed Record Request

To request a copy of your child’s Special Education Records, please complete the form below.

Special Education Records must be picked up in person from the Adams 12 Educational Support Center (ESC) located at 1500 E. 128th Avenue, Thornton CO 80241. A valid ID will be required upon pickup.

You will be emailed by a member of our staff when the records are available for pickup.  If you do not have an email address, please call 720-972-4147 to request records.

  • If your child is attending another school district, the new district should request these records.
  • Records are only available for students 23 years of age and younger.
  • This form is for Special Education records only.  Please contact the school directly for any other records needed.
