Medications
Administration of Medications at School
Per Adams 12 Five Star Schools District Policy 5420, the following is required in order for your student’s School Nurse or delegated staff to administer any prescription or over-the-counter medication. The only item a student may carry during the school day without a medication order from a health care provider is non-medicated cough drops.
- A district Medication Authorization form, (Español) must be completed annually by your child’s health care provider, along with a parent authorization signature. This form is to be used for all medications except asthma inhalers, Epi-Pens®, diabetic care, or emergency seizure medication.
- If your student needs to use an asthma rescue inhaler please have your student’s health care provider complete an Asthma Care Plan, along with the parent authorization signature and date.
- If your student needs an Epi-Pen® to treat a severe allergic reaction please have your student’s health care provider complete an Allergy and Anaphylaxis Emergency Care Plan, along with the parent authorization signature and date.
- If your student needs emergency medications for a seizure disorder, please have your student’s health care provider complete a Seizure Action Plan, along with the parent authorization signature and date.

Additional Considerations
- If your student is authorized to self-carry and administer a rescue inhaler during the school day please have the student’s health care provider complete a Self-Carry Contract (inhalers), (Español) in addition to the Asthma Care Plan. It is advisable that you provide back-up medication for the school health office in the event your child does not have his/her medication.
- If your student is authorized to self-carry and administer an Epi-Pen® during the school day please have the student’s health care provider complete a Self-Carry Contract (Epi-pen® ), (Español) in addition to the Allergy and Anaphylaxis Emergency Care Plan. It is advisable that you provide back-up medication for the school health office in the event your student does not have his/her medication.
- If your student is authorized to self-carry any other medications please have the student’s health care provider complete a Self-Carry Contract, (Español) in addition to the Medication Authorization Form.
** A parent or guardian must bring the student’s medication to the school. All medication needs to be provided to the school in the original over-the counter package or prescription bottle. The school cannot accept medication in any other container such as baggies or envelopes. All inhalers and Epi-Pens® need to include the original prescription box OR have a prescription label directly on the inhaler or Epi-Pen®
